Service Coordinator

The Boise City/Ada County Housing Authority is accepting applications for a full-time Service Coordinator. This is a grant-funded position. The Service Coordinator is responsible for connecting elderly and persons with disabilities to supportive or medical services provided by public agencies or private practitioners within the general community by providing general case management and referral services to all residents from a variety of needed services; researching cost and scope of services allowing residents to select from a variety of needed services; educating residents on available services; application procedures, clients rights, etc. and providing advocacy; communicating with service providers and residents to ensure needed services are received; helping residents build informal support networks; providing and coordinating training of residents in obligations of tenancy; and educating staff on the issues of aging in place and service coordination.

Knowledge of Gerontology, and of resources and services available to elderly and disabled residents in the Boise City / Ada County community; ability to communicate clearly and concisely, both orally and in writing; demonstrated ability to advocate, organize, problem-solve and provide results for the elderly and disabled served; establish and maintain effective working relationships with those contacted in the course of work. A Bachelor degree majoring in Social Work, Gerontology, Psychology or Counseling from an accredited college or university is preferred (appropriate work experience may substitute for education) and a minimum of 2 years of experience in social service delivery with senior citizens and non-elderly disabled.
